Reporting occurrences in accordance with Regulation (EU) 376/2014
Safety-relevant occurrences must be reported to the FOCA within 72 hours.
A list of reportable occurrences is contained in Implementing Regulation (EU) 2015/1018. Voluntary reports may also be submitted to the FOCA.
Both mandatory and voluntary reports (formerly known as SWANS) must be submitted via an ECCAIRS interface (special installation required) or on the EU’s Aviation Safety Reporting portal. This portal is in English but entries may be also submitted in German, French or Italian. The system automatically relays the reports to the FOCA. No other public authority or organisation has access to the reports. The FOCA has produced a guide for general aviation (GA) on how to use the reporting portal.
Anyone reporting an occurrence via this channel will receive a confirmatory email and a link to their report (where an email address is provided). The report is recorded and analysed by the FOCA. Measures are then defined on the basis of any safety risks identified. Furthermore, any lessons learned from the report are published on the FOCA website and by the «Stay Safe» campaign.
In accordance with Articles 10 and 11 of Regulation (EU) 376/2014, ‘interested parties’ listed in Annex II of the Regulation may request access to certain information contained in the European Central Repository. Requests are to be sent to the SRM office (see contact). Information may be used in accordance with Articles 15 and 16 of the Regulation.

Just Culture Declaration FOCA
The reporter does not experience any prejudice based on the information they have reported to the FOCA, in line with the reporting requirements. This principle applies, as far as no willful misconduct or serious lack of professional duty of care is evident.
Reporting Office ROJCA (Reporting Office for Just Culture in Civil Aviation)
The Reporting Office for Just Culture in Civil Aviation aims to strengthen Just Culture through the protection of the information source of an occurrence reporting in Swiss Civil Aviation. Individuals can inform the Reporting Office of alleged infringements regarding the protection principles by national authorities or their organisation.
The ROJCA does not replace the occurrence reporting system of the FOCA. Irregular occurrences that compromise or that could compromise air safety must be reported to the FOCA.
