RAM Continental engine TSIO-520-NB shut down

The issue (Report)

A serious incident occurred at Menorca airport during the take-off phase of a twin-engine Cessna 340A. The L/H RAM Continental engine TSIO-520-NB shut down 50 meters above the ground after take-off. Fortunately, the aircraft was able to land without damage immediately after the incident.

Cause

The cause of the engine shutdown was the breakage of the camshaft gear which caused the immediate engine shutdown.

The engine was overhauled by RAM Aircraft Limited Partnership, Waco Texas 76708 on 28/03/2019.

The broken gear partnumber 631845 should have been replaced during the engine overhaul with an improved camshaft gear partnumber 656818 to eliminate the possibility of gear tooth fracture regarding the CSB05-8D issued the 09/08/2005 with the last revision of the 22/08/2018.

In the list of Service Bulletins complied with at the time of overhaul (March 28, 2019), on TSIO-520-NB Left Engine s/n 290516-R, an obsolete version of this Service Bulletin (SB05-8B) is mentioned but has not been applied. Moreover, during the overhaul of the engine the latest version of this Critical Service Bulletin, the CSB05-8D, was already applicable. There might be a risk that the second engine of the affected aircraft (RAM Continental TSIO-520-NB) and other engines overhauled by RAM Aircraft L.P are affected by the CSB05-8D. Affected engines should be inspected to ensure they are equipped with CSB05-8D compliant camshaft gear part number 656818.

Measures

The BAZL has forwarded all relevant information from these safety-related incident reports to the respective organization responsible for the design of the aircraft, engine, propeller, parts or appliances. -> EASA / FAA

The BAZL is using this SAND* to inform the aircraft operators affected in Switzerland about a possible safety risk during the operation of the affected aircraft.
