Occurrence reporting

In accordance with Art. 20 ANA, all incidents, serious incidents and accidents involving manned and unmanned aircraft falling under Art. 4 Regulation (EU) No. 376/2014 must be reported via the reporting portal to the Federal Office of Civil Aviation (FOCA) within 72 hours of notification. The FOCA has produced a guide for general aviation (GA) on how to use the reporting portal.

In accordance with Art. 23 of the Air Navigation Act (ANA) in conjunction with Art. 17 of the Ordinance on the Safety Investigation of Transport Incidents (OSITI), accidents and serious incidents involving manned aircraft on Swiss territory, or involving aircraft registered in Switzerland which occur abroad, must be reported immediately to the STSB reporting office.

In case of doubt, STSB should be informed by telephone (1414 in Switzerland, +41 333 333 333 from abroad). The duty investigator will immediately make contact and decide on the further course of action.

Just Culture Declaration FOCA

The reporter does not experience any prejudice based on the information they have reported to the FOCA, in line with the reporting requirements. This principle applies, as far as no willful misconduct or serious lack of professional duty of care is evident.

 

Reporting Office ROJCA (Reporting Office for Just Culture in Civil Aviation)

The Reporting Office for Just Culture in Civil Aviation aims to strengthen Just Culture through the protection of the information source of an occurrence reporting in Swiss Civil Aviation. Individuals can inform the Reporting Office of alleged infringements regarding the protection principles by national authorities or their organisation.

The ROJCA does not replace the occurrence reporting system of the FOCA. Irregular occurrences that compromise or that could compromise air safety must be reported to the FOCA.

A new aviation reporting portal is in operation since the beginning of January 2021. This portal has to be used for all new reports. 

Note: We strongly recommend the use of the online form. The offline version via pdf is not mature and often generates error messages.

Follow-up reports: Use the "organization account" of your company for follow-up reports. If you do not have an account, send your follow-up reports to occurrence@bazl.admin.ch.
